This appears to be a spinoff of the x change computer game.
Here is an imperfect translation that indicates a chemical
sex-change if I read it right...

i tried it,
in fact , laoru has often health problems with weird sudden pain
crisis, one day after feeling very bad, he's took at the schol
doctor's office, where he gets a medicine that makes him feel
better
the day after, kaoru goes to school again, and gets bad again, so
he goes to the doc's again, but find nobody there, only the
medicine's borrle on a table... or so he thought, in fact it was
not the same medicine, but an experiment, and there kaoru gets into
a girl!
unlike in x-change, after a few tests the doc's explanation is that
kaoru was already a hermaphrodite before, without knowing it and
the "magic potion" activated his female chromosoms and he
transformed into a girl in one night
that's still impossible but at least that serves the plot , as the
doctor says, it sould be possible to make a cure , but kaoru is
100% real woman, just like he was (and can be) a 100% real man
because of his hermaphrodite nature
so will he choose to live as a girl? or as a guy ? will other
things happen and other peoples may get involved with doctor's
sex-change-medicine-experiments ? gotta try the game to know ;)
(also: no more pain crisis once he's a girl ;) could it be a hint?

for scores i'd say design, drawing style etc is just great:)
characters are also nice and full of life :)
but the game has much less possible choices than x-change for sure,
it's much more linear, but the changes are really affecting the
followings (hint: you still can save at the choice screen :p )
the music is not MIDI anymore, it's CD quality, but it's funny that
it still sounds so much "h-game" typical :o) i find it fun :)
what's bad?
the HORRIBLE censoring, you just can see no genitals of whatever
kinds, even during simple non-sexual naked senes :'( you'll see a
pixellated round shape (with BIG pixels, like sometimes 4 pixels in
total! :p)
that's the kind of boring thing that usually canish in the english
versions of the game, but i don't know if it's planned :'(
to bad because i found this game almost better than x-change in the
way that it's more sweet and natural, kaoru still speaks as before,
and is cutely shy also, he does not get a true nymphomaniac as soon
as he turns into a girl, hat's really cute :)
also , i haven't really seen true rapes like the x-change's school
bus (i don't like this part , beh :p)
kaoru is only forced by promises he made etc,but no violence :)
that's refreshing :)
(of course i haven't seen all the scenes so it may be anyway , but
i don't think it can get really different from what i saw..)
so it's a good game for h-games lovers, and for sex change lovers,
etc, but too bad that censoring is so strong :'( and also if you
want a real "GAME" you'll be frustrated, it's more like an
interactive movie,
ok, most of h-games are, but this one is surely more than x-change,
even if it's subtitle is "x-change alternative" ;)
